How Much Does Speech Therapy Cost? (With and Without Insurance)

Cost is one of the first questions every family asks about speech therapy — and one of the hardest to get a straight answer on. Here’s the straight answer.

Without insurance: typical private-pay rates

Across the U.S., private-pay speech therapy commonly runs in the range of $100–$250 per session, varying with region, clinician experience, and setting. Initial evaluations usually cost more — often $200–$500 — because they involve standardized testing, analysis, and a written report. At a typical once-a-week cadence, uncovered therapy can add up to several hundred dollars a month, which is exactly why so many families stall on getting help.

With commercial insurance

Most commercial plans cover medically necessary speech therapy — meaning therapy that treats a diagnosed condition (a delay, articulation disorder, apraxia, stuttering, and so on). Once coverage applies, your cost is your plan’s cost-share: typically a copay of $0–$50 per visit, or coinsurance after a deductible. Two things to check on any plan: whether there’s an annual visit limit, and whether prior authorization is required after a certain number of sessions. With Medicaid: usually $0

For children, Medicaid is the strongest speech therapy coverage there is. Under the federal EPSDT benefit, state Medicaid programs must cover medically necessary speech therapy for members under 21 — and families typically pay $0 per visit. The hard part isn’t coverage; it’s finding a practice that accepts Medicaid. We do, in all four states we serve: see Virginia, Colorado, Illinois, and Louisiana.

Three ways to keep speech therapy affordable

  1. Get benefits verified before you start. Any good practice will do this. If a clinic can’t tell you your expected per-visit cost in advance, keep looking.
  2. Use telehealth. Online therapy is covered the same as in-person under the plans we work with — and it eliminates the hidden costs: drive time, parking, missed work hours.
  3. Don’t assume Medicaid means waitlists. It often does at traditional clinics; it doesn’t at practices built for Medicaid access.
